Key Responsibilities:
Provide expert support and guidance to children with disabilities and their families.
Conduct comprehensive assessments to determine individual needs and develop tailored support plans.
Collaborate with multidisciplinary teams to ensure holistic care and support for children and families.
Advocate for the rights and well-being of children with disabilities within the community.
Maintain accurate records and documentation in accordance with professional standards and regulatory requirements.
Requirements:
Qualified Social Worker Registered with SCW.
Proven experience in supporting children with disabilities in a social work capacity.
Strong communication and interpersonal skills, with the ability to build trusting relationships with children and families.
Excellent assessment and case management skills.
Ability to work effectively within a multidisciplinary team and collaborate with external stakeholders.
