Are you a dedicated and compassionate Social Worker looking for an opportunity to make a real difference in the community? Randstad is seeking a talented individual to join our Community Wellbeing Team.

Role Overview:
As a Social Worker in our Community Wellbeing Team, you will work collaboratively with individuals, families, and partner agencies to assess needs, develop care plans, and coordinate support services.
You will focus on promoting independence, wellbeing, and social inclusion, ensuring that vulnerable adults receive person-centered support that meets their individual needs.
Key Responsibilities:
Conducting holistic assessments of individuals' social care needs, considering their strengths, preferences, and aspirations.
Developing and implementing care and support plans in partnership with service users, their families, and other professionals.
Providing advocacy and support to enable individuals to access appropriate services and resources.
Monitoring and reviewing care plans to ensure they remain responsive to changing needs and circumstances.
Working collaboratively with partner agencies, including health professionals, housing providers, and voluntary organizations, to coordinate support and facilitate seamless transitions between services.
Promoting safeguarding and ensuring that appropriate safeguarding procedures are followed to protect vulnerable adults from harm.
Requirements:
Qualified Social Worker registered with Social Work England.
Experience working within a social care setting, preferably with adults in the community.
Strong assessment, communication, and interpersonal skills.
Ability to work autonomously and as part of a multidisciplinary team.
Commitment to upholding the principles of person-centered practice and promoting independence and empowerment.
Knowledge of relevant legislation, policies, and procedures relating to adult social care and safeguarding.
