Are you a dedicated and experienced Social Worker seeking an exciting opportunity to work within a hospital setting? Randstad Social Care is currently recruiting for a Hospital Team Social Worker to join a dynamic team in Sunderland.

Role Overview:
We are seeking a compassionate and proactive Social Worker to join the Hospital Team in Sunderland. In this role, you will work collaboratively with multidisciplinary teams to provide holistic support to patients and their families throughout their hospital journey.
Key Responsibilities:
Conducting assessments of patients' social care needs, taking into account their medical condition, personal circumstances, and support networks.
Developing and implementing discharge plans in partnership with patients, families, and healthcare professionals to facilitate safe and timely discharge from hospital.
Providing practical and emotional support to patients and families, signposting to relevant services and resources as required.
Liaising with community-based services, including social care, housing, and voluntary organizations, to ensure seamless transitions for patients returning to the community.
Advocating for patients' rights and ensuring their voices are heard in decisions about their care and support.
Participating in multidisciplinary meetings and case conferences to contribute to holistic care planning and coordination.
Requirements:
Qualified Social Worker registered with Social Work England.
Experience working within a hospital or healthcare setting, preferably in a similar role.
Strong assessment, communication, and interpersonal skills.
Ability to work effectively within multidisciplinary teams and build positive working relationships with colleagues and external agencies.
Knowledge of relevant legislation, policies, and procedures relating to adult social care and safeguarding.
