Pay Rate: £35-£38 per hour
About the Role:
As a Social Worker in the Hospital Discharge Team, you will play a crucial role in facilitating safe and timely discharges for patients. Your responsibilities will include assessing the needs of patients and their families, coordinating with healthcare professionals, and ensuring that appropriate support services are in place for a smooth transition from hospital to home or other care settings.
Key Responsibilities:
Conduct assessments to determine patients' needs and develop care plans.
Coordinate with healthcare professionals, families, and community resources to ensure comprehensive care.
Provide advocacy and support to patients and their families throughout the discharge process.
Maintain accurate and up to date records of assessments, care plans, and interventions.
Collaborate with multidisciplinary teams to optimize patient outcomes.
Requirements:
Degree in Social Work or equivalent qualification.
Registration with the Scottish Social Services Council (SSSC).
Experience working in a similar role, preferably within a hospital or healthcare setting.
Should be under the PVG Scheme
Ability to work effectively as part of a multidisciplinary team
