Randstad is seeking a Qualified Social Worker to work within Lancashire County Council's Hospital Discharge Team. This full-time position offers a competitive pay rate of £37 per hour.

Facilitate the smooth and safe discharge of residents to suitable settings.
Implement various Discharge to Assess and re-ablement pathways to promote independence outside of the hospital environment.
Conduct assessments to identify immediate and long-term care needs, supporting individuals in achieving their care goals.
Collaborate with a multi-agency team to ensure effective, safe, and timely discharges across different hospital sites.
Conduct Mental Capacity assessments, participate in Best Interest Meetings, and attend MDT meetings as required.
Contribute to a Duty rota.Requirements:

Registration with Social Work England
Enhanced DBS clearance
Social Work Degree or equivalent qualification
