Locum Band 6 Pharmacist - Community Hospital, Dartford - £28 p/h

* £28 per hour

* 09:00-17:00, 22.5 hours – 3 days per week

* Locum 3-month contract

* Dartford, DA1

* Minimum 1 years UK experience working as a Pharmacist

We are seeking a skilled and dedicated Locum Band 6 Pharmacist to join a Community Hospital Team in Dartford. As a Locum Pharmacist, you will be responsible for providing pharmaceutical care and support to patients in the hospital setting. Your primary focus will be on ensuring safe and effective medication management, dispensing medications, and providing clinical pharmacy services.

Key Responsibilities:

* Dispensing medications accurately and efficiently according to prescriptions and hospital policies.

* Conducting medication reconciliation for admitted patients to ensure accuracy and safety in medication use.

* Participating in ward rounds and multidisciplinary team meetings to provide pharmaceutical expertise and support in patient care decisions.

* Providing drug information to healthcare professionals, patients, and their caregivers.

* Monitoring patient medication therapy for efficacy, safety, and adherence, and making recommendations for adjustments as necessary.

* Contributing to the development and implementation of medication-related policies, protocols, and guidelines.

* Participating in the training and supervision of pharmacy support staff and students as required.

* Collaborating with other healthcare professionals to optimize patient outcomes and ensure seamless transitions of care.

* Maintaining accurate documentation of pharmaceutical interventions, medication histories, and other relevant information.

* Adhering to regulatory requirements, professional standards, and best practices in pharmacy practice.

Qualifications and Experience:

* Bachelor’s or Master’s degree in Pharmacy from an accredited institution.

* Registered as a pharmacist with the appropriate regulatory body.

* Minimum 1 years UK experience working as a Pharmacist

* Prior experience working as a pharmacist in a hospital or clinical setting is preferred.

* Strong clinical knowledge and understanding of pharmacotherapy principles.

* Excellent communication and interpersonal skills.

* Ability to work effectively both independently and as part of a multidisciplinary team.

* Attention to detail and a commitment to patient safety and quality care.

* Flexibility to adapt to changing priorities and work schedules
