Prescribing Clinical Pharmacist
Location: Surrey
Start: May 2024
3x Days a Week

Are you a clinical pharmacist looking for a new challenge? A fantastic opportunity has come up for an experienced Clinical Pharmacist to work within a supportive Primary Care Network in Surrey.

This permanent role offers the flexibility to work across two sites, providing you with the opportunity to showcase your expertise in a variety of settings. As a successful candidate, you will have completed the CPP pathway, hold an IP qualification and possess the ability to take initiative and work autonomously with minimal supervision.

As an advanced clinical pharmacist, you will play a crucial role in providing high-quality pharmaceutical care to patients and supporting the wider healthcare team.

Responsibilities will include but not be limited to:
• Conducting medication reviews and providing clinical advice to patients
• Developing and implementing pharmaceutical care plans
• Collaborating with healthcare professionals to optimize patient outcomes
• Participating in clinical audits and quality improvement initiatives
• Prescribing medication and providing clinical advice to patients
• Conducting medication reviews and audits to improve patient outcomes

Skills and qualifications required:
• GPhC registration as a pharmacist
• IP qualification and completion of the CPP pathway
• Strong clinical knowledge and decision-making skills
• Excellent communication and interpersonal skills
• Minimum 3 years' experience within a similar role
• Must have primary care experience
