Job Description

Our client is seeking a Medical Notes Summariser/ Administrator to join a friendly GP practice on a flexible part time basis, based in Reading, Berkshire.

Main duties/key tasks:

• Checking incoming paper and electronic medical records and producing an accurate summary of the patient’s medical history
• Coding the information into the practice clinical system in line with QoF (Quality and Outcomes Framework)
• Managing Electronic records management workflow
• Checking and updating records in preparation for provision of data to other healthcare providers
• Tracking and chasing outstanding medical records
• Responding to requests from Primary Care Support England
• Organising workload to meet practice requirements
• Maintaining strict confidentiality in line with GDPR regulations & Practice Policy
• Follow and maintain practice policies, standards, and guidance
• Any other reasonable administrative task to assist the Practice

Job Responsibilities:

• A keen eye for detail is required so that all incoming patient records are accurately collated, summarised and stored on the patients computerised medical record and relevant information is coded in line with QoF.
• Accurately code missing immunisations to assist nursing team & update CHIS
• Review any GP2GP transfer of medical records and ensure they are accurately recorded and coded as per practice protocol and in line with QoF
• Deal with queries and questions relating to summarising and/or coding of medical records both from internal and external sources
• Locate patient’s medical records as needed by the clinical staff and request from the relevant source as required.
• Carry out administrative tasks relating to patient records, including filing, sorting and document management.
• Prepare reports as necessary
• Confidentiality with patients seeking treatment through them.
• Working with EMIS Web Surgery
