This is an opportunity for a Band 3 Community Support Worker to join the Integrated Outreach Team based in Keighley

The team is located at Meridian House Keighley.
We are a Monday to Friday 9 - 5 service.
Closing Date: 5th December 2022
Interview Date: 16th December 2022
The successful candidate will work with the multi-disciplinary team to provide holistic health and social care to working age adults. This will involve supporting people with a wide range of mental health issues, primarily psychosis. You will be working with team members to deliver planned care for a group of people who need an assertive outreach style approach.
The post holder will be required to work agile either from base, service user's homes or community site, may include some homeworking.
